Prescription Refills

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's prescription by submitting the following form. E-mail is checked daily Tuesday thru Friday before 1 PM. Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor, so please allow 48 hrs. for prescriptions to be filled and shipped. If you prefer to print this form, select the box below starting with Form-Rx Refill Form, print and fax it to us. Our fax number is 816-640-3156.

We must have a credit card number on file or we will contact you by phone for that information. Your prescription will be mailed to you. To make shipping more affordable, we have reduced our shipping fees for most items. Orders less than $100 will be charged $5 for shipping. Free shipping is available for orders over $100. There are some larger items that cannot be shipped.

NOTE: If you are ordering more than one item, please list them with 1, 2, 3, etc., in the box with Prescription to be filled.
